  The Illinois Department of Public Health (IDPH) provides, in English and Spanish, a Statement of Illinois Law on Advance Directives and DNR Orders and Living Will, Declaration for Mental Health Treatment, Power of Attorney for Health Care, and Do Not Resuscitate forms. All of these may be accessed on the IDPH website.
