2005 Patient Safety Learning Collaborative"Medication Reconciliation" The 2005 Patient Safety Learning Collaborative on Medication Reconciliation concluded with a well-attended Outcomes Congress in October 2005 in Bloomington. Participants in the Collaborative presented on key milestones for implementation of a medication reconciliation process, and shared their lessons learned, tools, and educational materials with the more than 70 organizations that attended the program. The diversity of the participating organizations was evident in the variety of solutions presented during the Congress. All presentations from the Outcomes Congress and other materials (forms, tools, policies, and educational materials) submitted by the participants, are now available on an interactive CD for use by other hospitals. The CD is being distributed free of charge to all attendees of the Outcomes Congress, and is available for purchase for $25 through the Chicago Patient Safety Forum.
