May 8, 2008 Changes in Hospitals’ Behavioral Health Reporting RequirementsPlease see below a letter from the Illinois Division of Mental Health (DMH), outlining hospital reporting obligations under legislation related to the issuance or revocation of a Firearm Owner's Identification Card (FOID) to a person who has been admitted to a mental health facility, PA095-0564 (740 ILCS 110/12). The new reporting requirements take effect on June 1, 2008. Under current law, hospitals are required to report to DMH on a monthly basis. Hospitals will now be required to report on a weekly basis. However, hospitals will be able to report electronically, using the Internet. In order to do so, hospitals must pre-register with DMH, no later than May 12, using the forms referred to in the attached letter. This form also is available on the DHS web site at www.dhs.state.il.us/foid. Please note that hospitals must designate an authorized user on their behalf, who must complete the form.
