Features and Benefits
Designed to
meet the recruitment needs of healthcare facilities, the AMR Employment
Application undergoes an annual review by labor relations attorneys. This is to
ensure overall compliance with equal employment opportunity regulations of
federal and state agencies. An ad hoc group of healthcare human resource
professionals assist in the review process.
The Application addresses issues of concern to personnel administrators
including:
- Americans with Disabilities Act
- Healthcare Workers Background Check Act
- Civil Rights Issues
- EEO Regulations
- Professional Licensing and Certification
Once the
review process is completed, AMR notifies all clients of the changes made to
the Application and the reason behind the changes.
The AMR Employment Application is available in both pre-printed and electronic
format (for web site posting).

Employment Applications are purchased by hospitals, physician clinics, nursing
and retirement homes, hospices, home care agencies, healthcare staffing
organizations, family service organizations, and diagnostic centers.
Front Cover Customization
Options
The front
cover of the Application, which contains a standard design, can be customized
with your organization's name and logo. Your facility also has the option to
specially design the front cover. This enhancement provides an inexpensive way
to strengthen your organization's professional image within your community.
Cover Styles Available:
- Standard (with or without facility name and logo
imprint)
Imprint area:
6"w x 1.5"h
- Special Order (call for details on specially
designed covers)
Imprint area:
7.25"w x 9.75"h
Imprint Color: Black, unless otherwise specified.
Artwork:
Please provide camera-ready artwork to size for custom imprinted covers. An
additional charge may be applied if artwork is not considered camera-ready or
is not easily reproduced by our graphics department. Please mail or e-mail
artwork to AMR along with your order.
Graphic Design Charges: If artwork is not camera-ready or you require assistance with your
design, graphic charges are $50/hour.
Customization Printing Charges:
- $25 set-up charge for first-time customized orders or artwork
changes on existing artwork.
- $25 for each additional color requested (black is the
standard imprint color).

Electronic Employment Application
The AMR
Employment Application is also available as an Adobe PDF file that can be posted on your
facility's web site. The file is an exact replica of the AMR pre-printed employment
application, providing a standardized look to your employment applications.
A web site-based employment application provides a great opportunity for you to
reach out to a larger field of viable candidates for your hiring needs. It may
also save staff time and expense by reducing the number of applications mailed
to interested individuals. By directing individuals to your website,
applicants will also learn more about your organization.
As with the AMR pre-printed forms, you have the option of using the standard
AMR cover or a customized cover (or you may omit the cover entirely). This
form was developed to work in conjunction with, not in place of, AMR pre-printed
applications.
The AMR Electronic Employment Application is available through an annual
subscription. A License Agreement (renewable annually) must be signed by all
facilities interested in subscribing to this product. The annual fee entitles
you to receive any updates to the form that occur during your licensing period
at NO ADDITIONAL CHARGE.

Order Placement Information -
Pre-printed Forms
Quantity: We recommend obtaining no more than
a 12-month supply of Applications in order to take advantage of our annual
review and enhancement process.
**AMR maintains a five-year running history of order placement to assist our
clients with their inventory management.
Minimum Order:
One box (125 applications/box)
F.O.B.:
Naperville, IL; shipping prepaid and added to invoice (shipped via UPS)
Sales Tax:
Charged on orders for taxable Illinois facilities. (Note: Illinois tax-exempt
facilities must supply their federal tax I.D. number on the order form.)
Shipping Time:
Two weeks from time of order (may be slightly longer for new artwork on
first-time or revised custom orders)
Payment Terms:
- First-time orders: PREPAYMENT required
- Repeat orders: Due upon receipt of invoice
Confirmation: AMR will call you to acknowledge receipt of your order and
to verify the total cost.
